Preferred Engineering, has facilitated numerous HAZOPs for highly varied projects, including refinery, polymer, petrochemical, LPG compression and storage, ship and barge loading/unloading systems, vapor combustion and recovery units. Primatech PHAWorks is the software used for HAZOP documentation. Project P&IDs are divided into nodes for team discussion. Guidewords of less, more, reverse, and misdirected are applied to the parameters of flow, pressure, temperature, level, and composition. Deviations are analyzed by the team and HAZOP worksheets and action items assignment resolution forms are generated by PHAWorks. Node design intentions are included in the worksheets. Facility siting checklists are utilized. Previous incidents are discussed and documented.   All action items are assigned risk levels by the PHA team using a severity/probability matrix.
